This system has earned its great reputation by offering a comprehensive continuum of outpatient healthcare services, including Family Medicine, Pediatrics, and Internal Medicine.The Social Worker will have the opportunity to coordinate, negotiate, manage and implement the care of complex patients throughout the facility to achieve quality and cost effective patient outcomes! Working collaboratively with interdisciplinary staff and working closely with the Social Work Leadership team – the LCSW will be fully supported.
