Description: Job Summary:We are seeking a compassionate Hospital Social Worker to provide support to patients and their families. The Social Worker will assist with discharge planning, coordinate community resources, and offer emotional support to help patients navigate the healthcare system.Key Responsibilities:Assess patient needs and develop discharge plans.Coordinate with healthcare teams to arrange post-hospital care.Provide emotional support and counseling to patients and families.Connect patients with community resources and services.Advocate for patient rights and access to necessary care.Qualifications:Master s degree in Social Work (MSW) required.Licensed Clinical Social Worker (LCSW) preferred.Experience in a hospital or healthcare setting.Strong communication and advocacy skills.

Salary: $50 – 58 per hour

Location: Lawrenceville, GA
