Description: We need a full-time Social Services Designee to join our team at Stonespring Skilled Nursing and Rehabilitation in Dayton, OH.You’ll assisted our Licensed social Worker by ensuring that the medically-related emotional and social needs of our residents are individually met and maintained.$15.50 to $19 Per HourYou’ll Be Responsible For:Assisting with gathering information from residents and family membersAssisting with the completion of the Integrated Personal Services Assessment and MDSAssisting with the discharge planning process by arranging community resources, alternative placement, etc.Attending Care Conferences as directed by the LSWAssisting LSW with any other case management tasks deemed necessary to the Department.Fulfilling basic documentation requirements for social services.Assisting (after training) in the BIMS and the PHQ-9You’ll Need To Have:Excellent communication skillsA High School Diploma and/or GEDHealthcare industry, social work, or other related experienceThe ability to multi-task in a fast-paced, high volume environmentExceptional organization skills with strong attention to detailA personable and professional demeanorComputer proficiency in Excel, Word, Power Point, and OutlookWe’re Proud To Offer You:
