The primary objective of this position is to improve access to healthcare services and reduce hospital readmission. This is a grant-funded position until 2025 year-end, strong possibility of future extensions.Qualifications:Required:Bachelor’s degree in healthcare administration, public health, or related fieldMinimum of 3-5 years of experience in public health or healthcare administrationStrong leadership and management skills with the ability to motivate and inspire a teamExcellent communication and interpersonal skillsAbility to analyze data and make data-driven decisionsFamiliarity with healthcare regulations and patient privacy lawsKnowledge of medical terminology and healthcare proceduresAbility to work collaboratively with clinical staff and other stakeholdersProficiency in Microsoft Office Suite and electronic medical records systemsScheduling Requirements: This role requires local travel/support between New Brunswick and Somerset as needed.Essential Functions:

  • Manage and lead a team of Community Health Workers to meet program goals
  • Travel between multiple hospitals and organizations in the region regularly
  • Develop and implement process improvement plans to increase patient engagement and compliance
  • Analyze and report on program effectiveness and recommend adjustments as needed
  • Coordinate team member schedules across multiple facility locations
  • Collaborate with clinical staff to ensure that patients receive timely and appropriate care
  • Regularly meet with team members to review performance and metrics
  • Provide regular supervision sessions with CHWs
  • Track and report on patient outcomes and the effectiveness of the program to senior leadership
  • Ensure that all Community Health Worker activities comply with healthcare regulations and patient privacy laws
  • Maintain accurate records of all Community Health Worker and patient interactions
  • Foster strong relationships with key stakeholders by participating in regular meetings to enhance quality services and promptly address requests
  • Connect with community organizations to maintain and facilitate strong relationships
