Description: Description of the role:University Home Care is seeking a compassionate MSW for our Hospice Home Care division in Livonia, MI. This position plays a crucial role in providing support and counseling to patients and their families during challenging times.Responsibilities:

• Conduct psychosocial assessments and provide counseling services
• Collaborate with interdisciplinary team members to develop care plans
• Advocate for patients’ needs and preferences
• Coordinate community resources and services

Requirements:

• Must have a Master’s degree in Social Work
• Licensed Clinical Social Worker (LCSW) preferred
• Prior experience in hospice or palliative care
• Strong communication and interpersonal skills
