Description: Works with the teams at Delta High School. Devoted to the delivery of primary care and behavioral health, with emphasis on prevention and health maintenance both physically and mentally. The integrated care therapist will utilize a variety of treatment approaches to work with a diverse child and youth population. Services include individual, but are not limited to, group and family therapy, social emotional group activities, skills groups, behavioral supports, crisis management, care coordination, consultation, and community outreach and support.
Principle duties and responsibilities:

  • Delivers and documents individual, group, family therapy, crisis intervention, and case management services to clients.
  • Conducts mental health status examinations and diagnostic assessment to establish differential diagnosis and treatment plans.
  • May deliver person centered, culturally sensitive evaluations, and trauma informed services as defined by position. These services may be delivered in various locations.
  • Works with clients and their systems to identify treatment goals and objectives.
  • Collaborates with all partnering organizations of A Kidz Clinic.
  • Maintains professional licensure and certifications as required for the program.
  • Attends all staff meetings, in-service meetings, peer reviews, supervision sessions, and other meetings as requested/required.
  • Files materials and documents into electronic health records.
  • Responsible for making appropriate appointments with psychiatric staff and collaborates with staff psychiatrists to coordinate client care plans.
  • Works cooperatively and constructively as part of an interdisciplinary team.
  • Responsible for keeping client information confidential by adhering to Colorado law and policies and procedures for Delta County School District.
  • Responsible for compliance with applicable federal, state and local laws; professional practice acts and ethical guidelines; policies and procedures of Delta County School District.
  • Responsible for supporting Delta High School mission and vision by practicing and modeling a culture of health and wellness.
  • Completes all administrative documentation and time sheets as requested/required.
  • Reviews and acknowledges Delta County School district policies and procedures on an ongoing basis.
  • Promotes and believes in integrated healthcare and promotes the mission of whole child healthcare.
  • Ability to relate to the public regardless of racial, ethnic and economic status.

The job holder must demonstrate current competencies applicable to the job position and other duties as assigned and required.

Education/Formal Training:

  • Masters level degree in one of the recognized mental health professions
  • Fluency in Spanish preferred; English required
  • Experience in a school based program preferred
  • Training in conducting diagnostic evaluation to establish differential diagnosis and treatment plans
  • Colorado State License or License eligible

Knowledge, Skills and Abilities:

  • A working knowledge of individual, group, family, and crisis treatment approaches and their application to clients with emotional, behavioral, and substance abuse issues.
  • Ability to produce written documentation and/or dictation of clinical work in a manner readily understood by other service providers
  • Ability to summarize service themes from a variety of client contacts and communicate suggested improvements for service delivery.
  • Ability to do differential diagnoses based on diagnostic evaluation, DSM 5, service planning, case management and treatment.
  • Ability to assess level of risk and make appropriate clinical judgement for resolution of client emergencies to effect a safe outcome
  • Basic computer and keyboarding skills
  • Ability to deliver services where the client’s needs are. This may necessitate travel within the community
  • Some knowledge of psychotropic medications and medical information relevant to mental disorders.
  • Ability to understand and utilize collaborative documentation when clinically justified and as requested by the client.
  • Ability to interact with people in a professional, and friendly manner.
  • Ability to work independently as well as cooperatively and constructively as part of an interdisciplinary team

Involves frequent contact with staff, patients, and the public. Interaction with others is constant and may be interruptive.

  • Often requires sitting and use of hands and fingers, to handle and feel

  • The noise level in the work environment is usually quiet to moderate.

  • Services are provided in a school-based health center.
